Registration Information and Discounts
Reservations for transportation, before-care and aftercare
should be made at time of registration. In May we will notify
you when our Parent Packs are available online. This packet
will include everything you need to know about camp and
then some! Included in the packet will be all of the necessary
forms that will need to be completed prior to your child
starting camp. Current health forms, the Florida State HRS-H
Form 3040 and the Florida certificate of immunization 680
Part A, are available from your pediatrician and must be
on file in the camp office. For the health and safety of
all campers and staff, campers will not be permitted to
attend camp without a completed medical examination and
required paper work. All forms must be turned in no later
than May 14, 2010.

Early Bird—5% discount
Be an early bird and register by January 31, 2010! To qualify
for the early bird discount, all fees must be paid in full
by January 31, 2010. Cannot be combined with Sibling Discount.
Sibling Discount—5% discount
A 5% sibling discount will be applied to the lower camp
fee for the second and subsequent sibling(s) attending any
Camp Shalom program. The discount will be applied to your
final balance. Our Sibling Discount Registration will be
held through March 30, 2010. To qualify for the sibling
discount, all fees must be paid in full by May 1, 2010.
Cannot be combined with the Early Bird Discount.
8 Week Discount—5% discount
Register for 8 weeks of camp prior to June 13, 2010 and
save!
Friends of the J—5% discount
Register for camp at any time and save 5%.
Discount will be applied to additional weeks added.

Financial Assistance
Limited financial assistance is available. Applications
for assistance will be available Feb 1, 2010 - April 1,
2010. For more information, or to receive a financial assistance

Registration Fee
Any families who register for camp:
•
By January 31 will not pay a registration fee
•
Between February 1 - March 31, will be charged a $50 registration
fee.
•
After April 1 - May 31, will be charged a $100 registration
fee
•
After May 31: will be charged a $150 registration fee
*Registration
fee is per family (not camper).
Payment Information
To reserve a space for your child, a $250 non-refundable
deposit is required. This deposit will be applied to your
child’s total camp tuition. Refunds: No refunds will
be given after April 30, 2010. Enrollment after May 1, 2010,
must be paid in full at time of registration.
