Circus Camp Letter	• This provides information regarding Circus Camp. For those participating in Circus, it is necessary to read this letter.
Health History and Pick Up Authorization	• This form provides camp with the necessary health information regarding your camper, gives camp permission to transfer your child in the unlikely event of an emergency, and informs Camp Shalom of the people who have authorization to pick up your child from camp each day.
Chugim Form	• Chugim are our new elective period. The chugim are assigned on first come basis, so please fill this form out and return it to the camp office as soon as possible. (not applicable for Tzofim or Tikvah)
P.I.C. Fridays	• Pizza and Ice Cream Fridays, what could be better? Download this form to find out more information and to register!
Authorization Form	• This form authorizes the JCC for several different items we will use throughout the summer. Please be sure to complete this form.
Special Needs Intake Form	• For those children with Special needs (Tikvah and Inclusion), this form in mandatory.
Carpool Flyer	• Includes procedures for drop off and pick up at camp as well as info about how to get a carpool number.
Adult Programs	• The JCC North will now be offering programs for you while your kids are in camp! Read out our Film Festival event, scrapbooking class and our Camp Mom Book Club!
Fitness Calendar	• Check out the fitness class calendar for the summer! Some classes have been specifically planned at Duncan during camp!
