Information |
• This gives the camp a little more insight
on your camper. Please complete this form.

| Health
History and
Pick Up Authorization |
• This form provides camp with the necessary
health information regarding your camper, gives camp permission
to transfer your child in the unlikely event of an emergency,
and informs Camp Shalom of the people who have authorization
to pick up your child from camp each day.

Permission Form |
• This form does or does not give the JCC permission
to take and/or use photos of your child.
|
| Group
Request Form |
• This gives you the opportunity to request
other children to be grouped with your child. It is necessary
for us to have this form no later than May 14th
to ensure your group request.

Authorization
Form |
• This form authorizes the JCC for several different
items we will use throughout the summer. Please be sure
to complete this form.

Carpool
Information Sheet |
• Please fill out this form to let us know how
your child is getting home daily and if they have a sibling
or your are carpooling with another family on a regular
basis.

Medication
Authorization Form |
• This form needs to be filled out by the parent
and signed by the doctor if medication of any type is
needed to be distributed during the camp day.

• For those children in Tikvah, this form in
mandatory.

Children’s
FEAT |
• It is mandatory for all Tikvah campers to complete
this form in
order to participate in the field trip to Children’s
FEAT.

Pump
It Up Waiver |
• It is mandatory for all Tikvah campers to complete
this form
in order to participate in the field trip to Pump It Up.
